All initial study documents can be submitted to Schulman via:
Visit the Forms page to download the most current forms.
Where a signature is required, please scan the signature page of the document into PDF format before submitting it electronically. If you are unable to scan the signature page into PDF format, you may fax it to Schulman at 877.686.2365

Please note: If mailing documents please include the informed consent document(s) on a disk or CD in Microsoft Word format. Schulman does not accept faxed submissions of new protocols.
