​Revised Informed Consent
All revisions to an Informed Consent (IC) must be submitted to the Board, including those not resulting from an amendment (e.g., an increase in compensation, telephone number change, and additional safety information). Please submit for review an electronic copy of the current Schulman approved IC with revisions indicated in Microsoft Word track changes or a copy of the current Schulman approved IC with handwritten changes. Revised ICs should be accompanied by the 
Protocol/IC Change Submission Form, Revised Compensation Submission Form or Revised Investigator or Site Location Information Submission Form depending on the basis for the revision(s).
