Approval from the Board must be obtained prior to mailing an informed consent document (ICD) to a subject. To request approval you must provide, in writing, your rationale for not presenting the ICD in person. If approval is granted, two (2) copies of the ICD should be mailed to the subject with a self addressed stamped envelope. When sending copies of the ICD, please include instructions for the subject to sign and date both copies and to return one copy to the research site. You must then follow-up with a telephone call to the subject in approximately three (3) days to ensure the ICD was received and understood. Upon receipt of the signed ICD from the subject, the person who conducted the consent discussion should sign and date the ICD noting that the consent discussion was via telephone. The entire process must be recorded in the source documentation.
