Each site must submit, as appropriate:
- Interim Study Status Reports
- Periodic/Continuing Review Study Status Reports
- Final Study Status Reports
If you are part of a multi-center study, use the Study Status Report Form to submit interim reports and to request study reapproval for your site. Use the Final Study Status Report Form to submit a final report.
If you initiated the study as the PI/QI, use the Single Site Study Periodic/Continuing Review Form to request study reapproval, the Study Status Report to submit interim reports, and the Site Final Study Status Report Form to submit your final report.
